Basically, Sakura is subjected to experiments and is stockholmed into willingly participating and, eventually, falling in love with Orochimaru. Sakura shows how smart and clever she is and Orochimaru can either return the sexual attraction or simply show favoritism in her favor. He can be genuine in his affections or simply manipulating Sakura. Some level of affection does need to exist.
What role Sasuke plays in this is up to you: he could worry about Sakura or he could claim not to give two shits. Entirely up to you on their relationship dynamic.
